In the 1980s, some Roto-Rooter franchises and company-owned locations began offering service over time and general service pipeline improvements in general to residential and commercial customers. Today, Roto-Rooter's 24-hour pipeline service represents a major part of the company's business. The company has more than 600 service locations operating across North America serving more than 90% of the US population in all fifty states and over 40% of the Canadian population. Roto-Rooter is the largest provider of pipeline and sewerage repair & amp; channel cleaning services in both countries.
In 2009, Roto-Rooter introduced a book titled Chilling Tales From The Porcelain Seat that featured "a true story about the weird & unexplained things that happened and the Roto-Rooter Heroes that can fix it. " Roto-Rooter asks plumbers all over North America to send a true story about the strange things they encounter at work such as strange items taken from toilets and sewer pipes.
2015 marks the company's 80th anniversary.
International franchise operations have been established in Canada, Japan, Philippines, Hong Kong, Indonesia, Singapore, Brazil, and South Africa.

Roto-Rooter remains the largest provider of water, drain and waterway repair services in the US and Canada. Companies do little or no new pipeline construction work. Its main work is to make pipeline repairs to existing commercial and residential pipe systems, unclogging and repairing drains and waterways and providing water damage cleanup. Roto-Roooter started providing water damage cleaning services in several locations owned by the company in early 2015. By 2017, all branch companies and many independent contractors and franchises provide water damage cleaning services.
Roto-Rooter Corporation, headquartered in West Des Moines, Iowa, maintains a manufacturing plant, which produces Roto-Rooter tools, engines, duct cleaning cables, and knives for the pipe and drainage industries. The company also produces two lines of commercial sewage treatment products, one line only available through Roto-Rooter service technicians and the other is a line of retail sewer care products sold in retail stores.
Roto-Rooter employees are mostly unionized. CEO of Roto-Rooter Group is Spencer Lee. CEO of Chemed Corporation, Roto-Rooter's parent company, is Kevin J. McNamara. CFO of Chemed Corporation is David P. Williams. Thad Reinhard is president of Roto-Rooter Corporation in West Des Moines. Rick Arquilla served as president and COO of the Cincinnati-based Roto-Rooter Services Company, and he appeared in an April 2010 episode of the CBS Undercover Boss reality show.

In December 2017, Roto-Rooter released a TV commercial introducing a new slogan, "We Do Both," to explain to the public that Roto-Rooter is doing pipe repairs and cleaning water damage. The ads, which start airing on television and in targeted internet advertising, show a curious neighbor, a Roto-Rooter plumber standing by the side of the road "what are you doing with Johnson's couch?" The Roto-Rooter plumber explained that Johnsons had pipe damage that flooded their homes and that he had repaired the broken pipe and now Roto-Rooter "cleans up the damage."

Science Fiction Comedy "Futurama" parodies the Roto-Rooter jingle in the episode "Why Must Be Crustacean in Love" A robot and an invisible singer recite the jingle as a robot movement with its extended support arm.

Similarly, in 1993, the ninth episode of the fourth season of the television series The Simpsons featured a parody of the Roto-Rooter jingle for Homer's snow plowing business called Mr. Plow. Jingle uses Roto-Rooter jingle melodies but the lyrics are changed to "call Mister Plow, that's my name, that name again is Mr. Plow!"
A Roto-Rooter service van is shown jumping over an open section of the bridge over the Los Angeles River in the 2015 movie The Wedding Ringer and one of the characters in the film depicts a Roto-Rooter service technician.
Roto-Rooter is featured on the Syfy Ghost Hunter show (2004-2016) as the two TAPS founders working for Roto-Rooter as a plumber in the company's Providence, Rhode Island branch. There are segments featuring Jason Hawes and Grant Wilson working as pipe technicians at their work in Roto-Rooter among paranormal investigations on the show. In addition, Roto-Rooter van is featured in the opening segment of each episode.
